Can anyone suggest an expert on running named patient expanded access programs in France? Is it possible to organize reimbursement?

Parexel and Quintiles are both quite experienced at running EAPs in France (i think they're called ATUs there). If you're looking for a consultant try dr.herve.laurent AT wanadoo.fr ... he worked for both companies and is living in france.

I'm a US based researcher looking to do a study that includes France, but am told I need to register the study with a central authority to disclose payment to doctors. How does this work?

There is a central body which I had to go through when I ran a program in France - not sure what it is called though

You effectively have to contact the "Conseil National de l'Ordre des Medecins" to inform them about the payments that are planned for investigators running studies (they usually ask to have the contracts done with investigators) and they give you back a reply (a few years ago the "approval" of the fees was not mandatory but strongly recommended to obtain, you should check now as there's a long time that i didn't work with france but the submission is mandatory)
